About Golden Pathways

Closing the gaps between visits

Golden Pathways Senior Care & Solutions was created to help close the gaps that can occur between appointments, providers, hospitalizations, medication changes, and day-to-day life at home. We provide an added layer of clinical support while working alongside the patient's existing primary care provider, specialists, home health team, pharmacy, caregivers, and other treating professionals.

Our goal is to make health care feel more connected. We help patients understand their care plan, monitor important health trends, identify concerns earlier, communicate meaningful changes, and keep the people involved in their care better informed.

Golden Pathways does not replace a patient's primary care provider or specialist.

We complement the existing care team through communication, monitoring, follow-up, and coordination.

Our Services

Support built around each patient's path to better health

Chronic Care Management
Service 01

Chronic Care Management (CCM)

Ongoing support for eligible patients living with multiple chronic health conditions. Our team helps maintain a personalized care plan, reviews changes in health and medications, supports health goals, coordinates with treating providers, and follows up between traditional office visits.

Remote Patient Monitoring
Service 02

Remote Patient Monitoring (RPM)

When clinically appropriate, connected medical devices can help patients and their care team follow important health information from home. Examples may include blood pressure, weight, oxygen level, glucose, or other appropriate physiologic information. The care team reviews relevant trends and uses the information to support treatment and care coordination.

Care Coordination
Service 03

Care Coordination

We help connect the moving pieces of a patient's health care. This may include communication with primary care, specialists, home health, pharmacies, caregivers, and other treating professionals; post-hospital follow-up; medication updates; referrals; and follow-through on identified care needs.

Telehealth and Virtual Support
Service 04

Telehealth & Virtual Support

When clinically appropriate, virtual care can provide convenient access to the Golden Pathways clinical team for assessment, education, care-plan review, and follow-up without requiring travel for every interaction.

Who We Serve

Support for adults managing complex health needs

Golden Pathways is designed for adults who may benefit from additional support managing chronic health conditions at home, particularly patients who have multiple diagnoses, frequent health care transitions, complex medication regimens, monitoring needs, or multiple professionals involved in their care.

Examples of patients who may benefit:

  • Adults managing two or more chronic health conditions.
  • Patients recently discharged from a hospital or experiencing changes in their health.
  • Patients who may benefit from appropriate home-based physiologic monitoring.
  • Patients whose care involves multiple providers, specialists, caregivers, or home health professionals.
  • Patients and families who would benefit from an additional point of coordination and education.

01

What is Golden Pathways?

Golden Pathways Senior Care & Solutions provides chronic care management, remote patient monitoring when appropriate, virtual support, and care coordination for eligible patients.
02

Does Golden Pathways replace my primary care provider?

No. Golden Pathways works alongside your primary care provider, specialists, home health team, pharmacy, caregivers, and other treating professionals.
03

What is Chronic Care Management?

CCM provides ongoing support between traditional visits for eligible patients with multiple chronic conditions. It may include a personalized care plan, health and medication follow-up, coordination, education, and communication with the care team.
04

What is Remote Patient Monitoring?

RPM uses an appropriate connected medical device to securely transmit certain health measurements from home so relevant information can be reviewed and used to support the patient's care.
05

Will someone be watching my RPM device all the time?

No. RPM is not continuous emergency surveillance and is not an emergency-response service. Patients should call 911 or seek emergency care for an emergency rather than waiting for a device reading to be reviewed.
06

Can my caregiver or family member be involved?

Yes. With appropriate patient authorization, Golden Pathways can include trusted family members or caregivers in communication and care coordination.
07

What happens after a hospitalization?

Golden Pathways may help review important discharge information, medication changes, follow-up needs, monitoring, and communication with the appropriate members of the patient's care team.
08

Does Medicare cover these services?

Medicare Part B covers eligible chronic care management and remote patient monitoring services when applicable requirements are met. Deductibles, coinsurance, or other cost-sharing may apply depending on the patient's coverage. Patients should contact Golden Pathways or their insurance plan with coverage questions.
09

Can I stop participating?

Participation is voluntary. Service-specific Medicare, insurance, billing, or equipment-return requirements may apply when services are discontinued.
